These Terms of Service ("Terms") govern your use of Everyday Forms, operated by Dishuu, Carleton Place, Ontario, Canada. By using the platform you agree to these Terms.
1. The Service
Everyday Forms allows creators to build and share forms and collect responses from responders. Responders do not need an account to submit a response.
2. Acceptable Use
You may not use Everyday Forms to:
- Collect sensitive personal data without a clear lawful basis (e.g. health records, financial account numbers, government ID numbers)
- Send spam, phishing links, or deceptive content
- Violate any applicable law or regulation
- Impersonate another person or organization
- Attempt to access systems or data you are not authorized to access
3. Creator Responsibilities
As a creator you are responsible for:
- The content of the forms you create
- Obtaining any necessary consent from your responders
- Complying with applicable privacy laws in your jurisdiction when collecting responses
- Keeping your login email secure
4. Intellectual Property
You retain ownership of the form content and responses you create. By using the platform you grant Dishuu a limited license to store and display that content solely for the purpose of operating the service.
5. Availability
We aim to keep Everyday Forms available but do not guarantee uninterrupted access. We may modify or discontinue features at any time.
6. Limitation of Liability
To the maximum extent permitted by law, Dishuu is not liable for any indirect, incidental, or consequential damages arising from your use of the platform. The service is provided "as is" without warranties of any kind.
7. Termination
We reserve the right to suspend or terminate access for violations of these Terms. You may stop using the platform at any time and request deletion of your data.
8. Governing Law
These Terms are governed by the laws of the Province of Ontario and the federal laws of Canada applicable therein.
9. Changes
We may update these Terms from time to time. Continued use after changes constitutes acceptance. The "Last updated" date at the top reflects the most recent revision.
